INSTRUCTIONS FOR CONTINUING SCHOLARS (AND STUDENTS FUNDED PREVIOUSLY AT ANOTHER LSAMP INSTITUTION):
 

Continuing scholars are required to reapply every semester. After you complete this application, you will receive an email with a Declaration Form acknowledging the receipt of your application.

• IMPORTANT: Sign this Declaration Form and return it to the H-LSAMP Program Director’s office. Your application will not be considered complete until this form has been submitted to:
